OP Ambulatory Pharmacist Clinician - Specialty Pharmacy (Per Diem)
The OP Ambulatory Pharmacist Clinician is an advanced practice pharmacist responsible for assuring optimal drug use outcomes in high risk, high cost or complex patient populations. She/He applies specialized knowledge of pharmacotherapy to direct patient care, teaching, and consultation. Participates as a contributing member of ambulatory care teams such as rheumatology, dermatology, neurology, infectious disease, oncology, nephrology, or pulmonology. Provides clinical, pharmacy, and medication expertise in each area including pharmacy operations, prior authorization support and medication access, and medication therapy outcomes.
Flexible Work Arrangement: This is a hybrid position with the opportunity to become fully remote. Schedule flexibility is available, with hours ranging from 8 to 40 per week based on the successful candidate’s availability and business needs.EDUCATION:
Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience preferred. Licensed pharmacist in the State of Vermont.
EXPERIENCE:
3 years of advanced practice experience in their area of expertise preferred or equivalent experience in ambulatory care or a clinical outpatient setting.
