We are seeking a compassionate and dedicated  Licensed Practical Nurse to join the team in the Clinical Intake Coordinator role.

Schedule: M-F role, occasional weekend availability required

Setting: On-Site at Bishop Spencer Place

The Work:

The Clinical Intake Coordinator facilitates the long-term care and rehabilitation process by receiving referrals from multiple referral sources within and without the St. Lukes Health System, performs initial evaluation of referral for long-term care and rehabilitation appropriateness under Medicare, Medicaid, and commercial insurance regulations, determines need for additional information from referral sources, communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals.

Support clinically related Marketing activities.

On-call rotation for referral management.

This position may require local travel.
