Certified Nursing Assistant Certified - Senior Care-PACE, PT, Day

Job Summary

Reporting to the Manager, PACE RN , the certified nursing assistant provides direct care as assigned and works in the center. Responsible for the collection of data related to participant status; performance of basic nursing procedures; ongoing collection and documentation of participant data; assistance with treatments and procedures; and assistance with maintenance of patient care equipment. May require access to storage areas where medications are kept and carry out procedures that require the use of non-prescription medications.

Bonus Eligible

This position is eligible for a $2,000 sign-on bonus through March 1, 2023 for external candidates only.
 

Accountabilities

  • Provides participant care based upon individualized plan of care as directed by the Interdisciplinary Team, including the participants' physical, emotional needs, cultural, spiritual, psychosocial, and educational needs. - 60%

  • Documents and reports patient status and condition.  Maintains and ensures accurate maintenance of participants' medical records and treats records as confidential. - 20%

  • Demonstrates effective communication skills in collaboration with other Interdisciplinary Team members to ensure optimal functioning of the center. - 10%

  • Identifies opportunities for process improvement and participates in on-going process improvement activities. - 10%

Supervisory/Management Responsibilities

This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • High School Diploma or equivalent - completion of a Nursing Assistant or Patient Care Tech program

  • 1 year - Experience working with a frail elderly population

Required Certifications/Registrations/Licenses

  • CNA - NURSING ASSISTANT - NCNA

In Lieu Of The Minimum Requirements Noted Above

In lieu of school verification of a nursing assistant training course, will accept current or former South Carolina CN A certification as proof of completed state required training at time of hire.   

Knowledge, Skills or Abilities

  • Basic computer skills

  • Knowledge of office equipment (fax/copier)
