Job Description:

PRN, Days - Up to 4 Days to be Worked per week

• 5 Hours per Shift

• Start Time of shift - varies between 8a - 2p

• End Time of shift - varies between 1p - 8p



Job Summary
• Responsible for coordinating and assessing all patients referred to intensive outpatient programming.
• Coordinates the approval and level of care determination in cooperation with the psychiatrist, patient/family and insurance verification.
• Coordinates and facilitates outside referrals and placement if needed

  • Participates in the formulation of a treatment plan, implementation and evaluation 
  •  Implement therapeutic, evidenced based group therapy
    and psychoeducational classes to meet the needs of the patients
  • Provide brief therapeutic counseling
  •  Assist patients with paperwork needed for FMLA and 
    Short-term Disability in order to be away from work while in treatment 
  • Develops and maintains professional relationships with
    treatment team members, hospital and medical staff and community partners

Minimum Education, Experience, Training and Licensures Required

  • Master's Degree required
  • Active Kentucky License: LCSW, LPCC, or LMFT
  • 2 years of job-related experience required
  • Experience working in a hospital setting; knowledge of substance use disorder treatment; experience in group therapy preferred
